The HKPR District Health Unit sends parents an email or text message at 6- and 12-months post-partum regarding an Infant Feeding Survey. The email or text message link will be similar to this:
Dear
When you first had your baby, we either spoke to you or sent you a letter about our infant feeding survey that will help us to improve services for mothers and babies in your area. At the time we spoke to you, you agreed to participate and provided us with your email address so that we could email you the link to the survey. If you received only our letter and did not call to decline your participation in the survey, then we assumed that you wish to participate and would like to thank you.
Just as a reminder, participation in this survey involves answering some questions about how you have been feeding your baby and some questions about you. If you agree to participate you can choose to end the survey at any time, and you can also choose to skip any question that you do not want to answer. Please be assured that if you end the survey at any stage, it will not affect the services that you receive from the Health Unit. This survey is anonymous and the answers you provide will be kept private and confidential. Your name will never be used in any reports about the results of the survey.
If you have any questions about the survey link you have received, please call the Healthy Families Information and Support Line 1-866-888-4577 ext. 5003
