Personal Service Settings
Personal Service Settings offer or specialize in services such as: tattooing; body and ear piercing; haircutting/hairdressing; manicures and pedicures; waxing; facials; eyelash extensions and more.
Exposure to blood or body fluids from these procedures can lead to bloodborne infections such as Hepatitis B, Hepatitis C and HIV. The HKPR District Health Unit inspects these facilities once a year, and on a complaint basis, to assess infection protection and control practices, as well as cleaning, disinfecting and safety measures.
As an owner/operator, it’s the law in Ontario that you must provide written notice to the Health Unit about your intentions and location at least 14 days before:
- Opening a personal service setting;
- Offering additional services at a personal service setting; or
- Starting construction or renovation of a personal service setting.

This form is to notify the Health Unit of a new personal service setting, change or addition to services, change in location, renovation or new ownership.
